DIRECTIONS FOR THE CLOCK HOURS FORM: Please download and complete the application form. Click on the submit button to Email the form. Please provide complete information. Attach a brochure, registration form, announcement, etc., publicizing the activity and the time(s) instruction will take place. Attach additional information (agenda, outline, syllabus, etc.) regarding the objectives/outcomes/content of the activity if necessary. You can also print the form and mail the information to the address below.
